Program terms

NDL Referral Partner Program Terms 1. Partners earn rewards for new dental offices that sign up through their links. 2. Activation bonus: paid once when a referred office reaches the activation threshold in paid cases within its reward window. 3. Monthly rewards are earned per full block of paid Plus and Classic cases, subject to the monthly cap. 4. Rewards are held for 30 days before approval. Minimum payout applies; smaller balances roll over. 5. Referrals of your own office are not eligible. Next Dental Lab may review and reject any referral. 6. To receive payouts you must submit a signed IRS Form W-9 and have it approved. Rewards still build up before then. 7. Next Dental Lab may change these terms; you will be asked to accept any new version.
